The average direct primary care membership costs $138 per month, with a median of $100. That’s a flat fee for unlimited primary care visits, direct messaging with your doctor, and often wholesale-priced labs and medications. For everyday care, it usually replaces the copays, deductibles, and surprise bills that come with insurance-billed visits.

Methodology
DirectMedicine maintains a directory of direct primary care and cash-pay practices. For this report we extracted membership prices that practices publish on their own websites, then aggregated the 517 practices with a clearly stated monthly membership fee between $10 and $2,000. City and state figures include only markets with at least three priced practices. Prices reflect what practices published as of 2026-07-08 and change over time; always confirm current pricing with the practice directly.
